Everton Free School and Football College offers many other specialist routes for young people including Arts, Culture and Digital Media pathways. Students are referred to the school by other schools, academies and local authorities.
About the opportunity: We have an exciting opportunity for a Post 16 Transition Worker to come and join our Everton Free School and Sixth Form College and join a specialist taskforce who will support some of our most hard to reach students.
This role will play a crucial part in the success of our programme, and a Post 16 Transition Worker you will give young person(s), and their families the support and opportunities to develop social skills, gain qualifications, improve health and well-being and have new experiences in a safe and friendly environment.
We want to raise the aspirations of our young person, with a clear goal of them having a safe transition in to adulthood and reaching their full potential. You will focus on personalised support and tailoring our services to the needs and aspirations to those we support enabling them to fulfil their potential and achieve their own life goals; linking in with other organisations to provide quality wrap around support, ensuring all aspects of life are covered.
Other key responsibilities within this role include:
Making contact with parents and school/college to request prior data and background information and liaising with home and school regularly sharing information, planning meetings and visits and formulating risk assessments
Supporting learners with independent study
Meeting with students to find out their wishes and feelings around going back to school and then onto college or other post 16 provision
Formulating transition plans in conjunction with the student, parents/carers, the school/college and the team. This often includes facilitating change of school or seeking alternative provision
Who we are looking for: We are looking for a dynamic individual who has experience working with secondary age pupils within educational settings to take on the role of transition worker with specific responsibility for post 16 and NEET.
Ideally our successful candidate will have experience within a children’s social care or education setting, along with knowledge and understanding of working with the most vulnerable children in need who require intensive support and safeguarding.
