About the opportunity: We have an exciting opportunity for a Health & Wellbeing Coordinator (Carers and Older Adults) to join our expanding Everton in the Community Team.

This role will be primarily based from Everton’s People’s Hub but will also cover various locations across the Liverpool City Region.

The Health & Wellbeing Coordinator (Carers and Older Adults) will implement and coordinate a variety of effective carer and older adult projects. Working collaboratively with partners to identify individuals who meet the program’s inclusion criteria as well as the efficient transition of referrals from partners into it.

Other key responsibilities will include:

The coordination of engagement activities/events to promote and establish referral pathways into a range of carer and older adults support projects.

Developing effective relationships with a range of people from different backgrounds and ages to enable them to access EitC programmes.

Liaising with EitC’s GP and health promotion team to organise baseline testing and a lifestyle support package (e.g., health checks, healthy eating, physical activity assessments). Ensure that all participants have a pathway to access wider EitC programmes

The coordination and delivery of a range of activities that aim to improve the health, wellbeing, and quality of life of older adults and carers of all ages.

Traveling independently to reach various sites across the Liverpool City Region to attend sessions and meetings including at times working evening and weekends.

Who we are looking for: Essentially we are looking for an individual who has a genuine passion for supporting people to improve their health and wellbeing and the ability to display high levels of patience and empathy whilst engaging with vulnerable groups.

Other key Knowledge and Skills will include:

Level 2 Award in Health & Social Care (or equivalent)

Safeguarding Children and Vulnerable adults certificate

Valid First Aid Certificate

Full, clean UK drivers license

Willingness to work weekends and in the evening.

This role will be offered on a 12 month fixed term basis
