Role description

Our Health and Wellbeing Activator will organise and deliver key aspects of programmes within our Community Health portfolio including our new Bluebirds FIT FANS project, delivery of which will be evening and/or weekend based. Working closely with our Disability & EDI Coordinator to deliver programmes, measure outcomes and seek new opportunities within the Health & Wellbeing sector.


Responsibilities

» To lead on the delivery of the Bluebirds FIT FANS weight management programme.


» To use the interest of football to attract adults aged 35-65 to a 13-week healthy lifestyle multi component programme, delivered in multiple cohorts throughout a 12 month period.


» Record and report on all project engagement using VIEWS (impact reporting system).


» Ensure weekly collection of outcome and progress data.


» Regular Impact Stories from those completing the 13-week programme and tracking individual progress for a further 6 months beyond delivery.


» To work as part of a staff duo to deliver the programme mainly out of Cardiff City Stadium.


» To report to line manager with weekly updates on the recruitment, retention and delivery of the programme.


» Effective use of the Rota Cloud scheduling system to ensure safe delivery of sessions.


» To attend termly team briefings, half year and end of year appraisals with line manager.


» Effective use of the Breathe HR platform to request annual leave, record training and sickness.


» To be the lead staff member for actively promoting, recruiting and then retaining beneficiaries to take part in the programme. (30 per cohort for 6 cohorts in 2023)


» Effective use of the XERO app for mileage and expenses claims relating to programme delivery.


» To work with local partners to ensure effective promotion of programme.


» To manage and build effective partnerships with key stakeholders to ensure a consistent recruitment of beneficiaries are signing up to programme.


» To ensure clear and concise information is shared internally with line manager and during team briefings.


» Regular communication with other CCO’s to learn and share best practice.


To maintain the highest standards of safeguarding and health and safety on all projects to ensure people are kept safe.


» To use the MyConcern platform to record any safeguarding concerns.


» To complete and update risk assessments, accidents and incident forms where required.


» Attend training events, conferences and scheduled learning & development sessions to enhance knowledge and skills relevant to your post when asked.


» Fulfil deadlines set by line manager and key partners with regard to reporting of key performance indicators and other targets.


» To develop strong positive relationships with beneficiaries, key partners and community members.


» Attend and participate in meetings with Foundation staff, programme funders and other meetings when required/agreed to do so.


» Promotion of company values & behaviours across the organisation.
