Working in disadvantaged and deprived areas of seven West London boroughs with many local partners involved and delivering to around 23,000 participants, the Trust wishes to support local communities and provide opportunities for participants to make positive changes is their lives. Established in 1994 and obtaining charitable status in 2009, the charity is focused on:• Positive physical & mental health• Supporting young people to reach their full potential.
QPR Community Trust has a wide range of projects including The Tiger Cubs, Extra-Time over 60s club, QPR Primary Stars, Soccer Schools, Kicks, Healthy Eating Projects plus many more
Role Summary:This role is responsible for providing a 1:1 youth work mentoring service supporting a caseload of 10 young people every 6 months (on average), twice a year. They will work closely with partner agencies and their local NHS Trust to recruit young people on to programme, identify suitable progression routes and development opportunities that focus on the following to support young people including but not limited to, health and wellbeing, life skills and education and employability.

Main Duties and Responsibilities:• Recruit participants on to programme to be assessed by CAMHS clinician before starting programme• Ensure that all young people are contacted within two working days of the CAMHS handover meeting• Meet with the young person and co-design a personal plan based around Goal Based Outcomes• Develop positive relationships with young people through a variety of youth work styles/methodologies• Support young people to access internal and external programmes in line with their needs and interests• Liaise with project leads to ensure young people are fully supported within their chosen programmes including support with resources• Deliver mental wellbeing support for young people and manage a caseload, providing support at suitable times for the referral either face to face or virtually• Develop and promote support for parents/guardians/carers so that they feel more confident in supporting their child/young person with their mental health and wellbeing• Establish, maintain, and develop effective partnership working with stakeholders including relevant external agencies and organisations• Complete monitoring, evaluation and session recordings using internal systems, including contributing to the project monthly reports via Upshot as directed by the Advantage National Project Manager• Project manage QPRs Advantage’s high-quality mentoring provision that meets the needs of young people and support agencies• Ensure that all delivery, for which you are responsible, complies with all health and safety and safeguarding requirements including the production of individual risk assessments• Attend all scheduled supervision, meetings, and training events• Produce reports to demonstrate movement and results against strategy and KPIs• Take ownership for comments made by young people and alliances to improve the service and standards provided by the team• Foster a culture for excellence in safeguarding and health & safety• Produce reports to demonstrate movement and results against strategy and KPIs• Collation of data to help support all monitoring and Evaluation functions• Support mentors to ensure Safeguarding Minimum Standard Criteria is met• Commit to a programme of continuing personal development (CPD) • Ensure all activities are delivered to the standards outlines in the Trust/Club’s Equal Opportunities, Health and Safety and Child Protection policies.• Record and store sensitive data in line with the QPR Trust’s GDPR policies• Participate in the appraisal process.• Undertake every opportunity to promote a positive image of QPR in the Community Trust and Queens Park Rangers Football Club.
